21-338m Prior Authorization Reminder for Outpatient Therapy Visits

Be sure to get prior authorization after 12 combined visits

California Health & Wellness Plan (CHWP) fee-for-service (FFS) providers must request prior authorization for the below outpatient therapies/rehabilitation services after 12 combined visits.

• Physical therapy
• Occupational therapy
• Speech therapy

Visits 1-12 do not require authorization.

Prior authorization requirements are available on the CHW website.

Providers participating through the Community Care Independent Practice Association (CCIPA) must contact CCIPA, follow CCIPA’s prior authorization process and use CCIPA’s forms.
