Information on Member Transportation Through Logisticare for 2017

As we come to the beginning of the New Year, we would like to remind our providers that California Health & Wellness members are eligible for transportation for their medical appointments.

Every New Year each member starts off with three courtesy rides. Once a member has used these three rides, a Physician Certification of Services (PCS) form will need to be completed by the Primary Care Physician and/or staff to certify the appropriate level of transportation required to meet the member’s medical needs in order to get to their medical appointments. 

If you have any old PCS forms from 2016 they will become invalid in 2017 and will need to be renewed using the updated PCS form available on the California Health & Wellness website under Provider Resources.
